ITU Forms

Online Forms:

• Generic E-mail Accounts
  Generic E-mail accounts are e-mail accounts in which the account name is not linked to an individual's name but is instead linked to a campus organization, club or department. These forms will allow users to request a generic e-mail account, sponsor change, sponsor change if the previous sponsor is gone, and change of authorized users.
• Generic Calendar Accounts
  Generic Calendar accounts are calendar accounts in which the account name is not linked to an individual's name but is instead linked to a campus organization, club or department. These forms will allow users to request a generic calendar account, sponsor change, sponsor change if the previous sponsor is gone, and change of authorized users.
• MESA Accounts
  MESA is the new computer network system of services that takes advantage of the network's components to give users resources beyond what a stand-alone computer can provide. These forms willl users to request a MESA account or MESA share folder.
• Computer and Software Installation
  These forms will allow users to request the installation of computers, peripherals (printers, PDAs, smart phones), and software.
• Data Jack Activation
  This form will allow users to request the activation of data ports.
• Resident Network Device Registration
  This form will allow users to register a network enabled device (iPod Touch, iPhone 3G, Xbox 360, Playstation 3, Wii, etc) onto the Mason Resident network.
• Resident Technician Request
  This form will allow users who live in the Residence Halls to request service from the Resident Technicians.

Student Forms:

• E-mail Account Activation/Password Change (MEMO)
  This site will allow first-time Mason E-mail users to activate their accounts or current users to change their Mason E-mail password.

  Tell us what you think about the level of service we provide you. We welcome all suggestions and comments.

  This site will allow first-time mason/osf1 users to activate their accounts or current users to change their mason/osf1 password.
• Partition Checklist
  This checklist can be used by residence hall students who wish to repair their own computer if they are taken off the network for worm/virus/trojan activity. (It can only be used for a first offense. See checklist for details.)
• Patriot Web PIN Problems
  For students who have forgotten their 6-digit Patriot Web PIN.

  These are forms if you need new telephone service, need to change your current service, moving, or need to disconnect service.
• Wireless Guest Account Request
  Complete this form if you need a wireless Guest account.
